Application Guidelines

• This application should be completed by a parent or guardian. 
• Applicants are required to take the High School Placement Test for admission consideration. The application will not be complete without an exam score. There will be a single exam date: Tuesday, July 15, 2025, at Crockett Midtown High School of Science and Medicine, 571 Mack Ave. 
• All deadlines are final, including application deadlines, and, if applicable, enrollment deadlines.
• Please review your application carefully before submitting it. You may save the application in progress and return to make as many changes as necessary prior to submitting it.
